Where is the case still used? In the only place I can see it is when you hover over the cover in the player. For consistency with the browser I would remove it.
Does it appear somewhere else?

In the download cover dialog box.

IMO It doesn't look bad there, but for consistency it may be better to not show it.

It was hard to make a choice on this one so I voted for the option to disable it.

But, along with the option to disable it, I was also thinking maybe you should include the option to disable the mouse over image as well, as without the cd case look, it just wont look very nice on mouse over. Maybe its only me though?

any chance for a screen shot showing the proposed change?
fyi I voted for the option, I think I know what you are talking about, and I like my current layout as is.

Ok I close the poll. And the most voted is 'Give the option to disable it' with 22 of 40 votes. 2nd comes 'Yes please' with 13 votes and 'No thanks' received 5 votes.

I will try to keep it giving an option to disable it.
